To our knowledge, this is the first economic evaluation of NPEP in Australia. We performed a retrospective cost analysis of a populationbased observational cohort of 1601 NPEP patients between 1998 and 2004 in Australia. Costs were adapted to reflect current practice and combined with effectiveness measures to calculate the cost per seroconversion avoided. A healthcare provider perspective was used. Treatment costs included costs for consultations and outpatient visits during the sixmonth follow-up period, diagnostic tests and drugs. Other costs incurred at the state level for NPEP support services were also considered. NPEP was provided through hospital outpatient clinics 57% of prescriptions ; , general practitioners 22% ; and sexual health clinics 21% ; . In order to account for differences in terms of NPEP protocols and cost structures, we modelled the costs of each treatment setting separately and differentiated between two large Australian hospitals. The average NPEP cost per patient was A16, of which A8 52% ; was for drugs, A1 21% ; for consultations, A5 14% ; for pathology, and A2 13% ; for other costs. Sexual health clinics, GPs and hospital based services had a similar average cost per NPEP patient. Overall, the cost per seroconversion avoided was A, 740, 134 in our base case analysis. It was A, 089, 774 after unprotected receptive anal intercourse UAI-R ; and A0, 617 after exposure to a known HIV positive source. These ratios were very sensitive to assumptions about the risk of HIV transmission after UAI-R. With a rate of 2.76% instead of 0.82%, the average cost dropped to A0, 710 per infection avoided, A3, 773 after UAI-R exposure, and A1, 219 after exposure to a known HIV positive source. Although 88% of infections avoided were after UAI-R, 45% of the costs were incurred for other types of exposures where the cost-effectiveness ratio was over A million per infection avoided. Considerable differences in lameness distribution exist between racehorses and non-racehorses Sport Horses [SH] ; . In the SH stress related bone injuries are virtually non-existent. Acute, traumatic cortical and subchondral injuries do occur and are usually related to external trauma such as hitting a rail, falling on course, taking a misstep during competition or from injuries while turned out. Old racehorse injuries may become important in ex-racehorses being used for other sporting activities, but in many horses with osteoarthritis OA ; and osteochondral fragmentation of the carpus is well-tolerated by horses performing at slow speeds or jumping low fences. Chronic, degenerative conditions such as OA of the distal and proximal interphalangeal joints, navicular syndrome, OA of the metacarpo MCP ; and metatarsophalangeal MTP ; , OA of the distal hock joints and medial femorotibial joints are important sources of pain in the SH. Lameness abolished by palmar digital analgesia is the most important source of problems in the SH. Advanced imaging using digital radiography, bone scintigraphy and magnetic resonance imaging MRI ; has allowed us to characterize lameness problems in the SH and design management protocols based on specific diagnoses. The suspensory ligament and associated structures are extremely important, particularly in jumpers and dressage horses. Hindlimb suspensory desmitis is hugely important in older Warmblood or other heavy breed horses, in particular, in those with straight in the hock conformation. As in any horse in active competition the SH is affected by numerous gait abnormalities, both primary and secondary. Compensatory lameness is an important concept to recognize and leads to numerous co-existent injuries in the SH as in all horses ; . Often the SH is affected with bilateral lameness, which if relatively symmetrical, can be difficult to identify during lameness examination. Hindlimb lameness can be confused with forelimb lameness. Below I have outlined my thoughts about recognition of forelimb and hindlimb lameness during movement, the possibility of defining from where pain is originating based on limb flight, the value of diagnostic analgesia, the differences between racehorses and the SH using OA of the fetlock joint as a model and end with a discussion of one of the most important issues in the SH, suspensory desmitis. During the lecture I will use numerous examples with video segments to illustrate gait deficits seen in SH with not only common lameness problems but in some in which perplexing lameness questions exist. This performance of Chile is remarkable considering that commercial salmon farming in the country started only a little more than 20 years ago. Unlike many other current exporting countries e.g. Norway, Canada, the UK and USA salmon is not a species native to Chile and thus its domestic market, human resources and knowledge about salmon all essential factors for building competitiveness did not exist from the beginning. Given such conditions, why was it possible for Chile, a developing country, to become the top exporter of farmed salmon? This paper attempts to explain how Chilean firms obtained competitiveness, by focusing on its organizational capabilities. Observing the organizational evolution process of the Chilean salmon industry is used to illustrate how the agro-food sector can be an innovative sector contributing to the formation of technology as well as human resources. Many factors are considered important in improving export performance, such as natural endowments and cheap labour; however, this paper takes the stand that mere existence of these factors did not make Chile a top exporter of farmed salmon. Instead the paper illustrates how the organizational structure was capable of flexibly adjusting to the conventional factors needed to obtain competitiveness through meeting certain necessary conditions.
